This role is ideal for clinicians who value patient-centered care, professional collaboration, and clinical growth, and who understand the unique responsibility and privilege of working with geriatric populations, including individuals with dementia and complex medical needs. Lymphedema certification is required for this role. In this position, you will: Evaluate and manage edema impacting ADLs, mobility, and skin integrity Implement skilled interventions including compression, positioning, and manual lymphatic drainage (MLD) Reduce risk of complications such as skin breakdown and infection Provide education to patients, families, and facility staff on long-term management strategies This specialization is essential to supporting safe, effective, and holistic care within our interdisciplinary model. New Graduate Occupational Therapists New graduates are encouraged to apply if you: Have a genuine interest in geriatric and dementia-informed care Are seeking a structured environment with guidance and mentorship Value feedback, communication, and professional development Are comfortable learning within a facility-based, team-centered model of care Success in this role is driven less by years of experience and more by: Professional maturity Emotional intelligence Teachability and openness to learning Alignment with our values and approach to care Core Clinical Responsibilities Provide skilled occupational therapy services to geriatric patients across varying diagnoses and functional levels Adapt treatment plans based on daily patient presentation, including cognition, stamina, mood, and medical status Support functional independence, safety, participation, and quality of life Deliver care that may be restorative, wellness-focused, or maintenance-based depending on patient needs Understand the cycle of care and how each visit fits into the patient's broader plan Emotional Intelligence & Patient-Centered Care Recognize and respond appropriately to changing patient emotional states Communicate effectively with: Patients Families and POAs Facility staff Interdisciplinary therapy teams Determine what information is essential to share with families and facilities, recognizing that care needs are not "one-size-fits-all" Preserve patient dignity, autonomy, and trust in all interactions Collaboration & Communication Work collaboratively with PTs, SLPs, nursing staff, aides, intake, and leadership Participate in coordinated care planning and interdisciplinary communication Accept feedback and align with organizational care standards Communicate proactively within facilities to support continuity of care and appropriate referrals Documentation & Professional Accountability Complete timely, ethical, and defensible documentation Understand unit-based billing and productivity expectations Adapt to documentation platforms, including WebPT and EPIC Balance efficiency with clinical quality, safety, and compliance Support, Supervision & Mentorship At Therapy at Home, supervision and mentorship are intentional, structured, and relationship-based.
