Job Summary We are seeking a dedicated and compassionate Mental Health Rehabilitation Specialist to join our dynamic healthcare team. Duties Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, to determine individual needs and treatment plans. Develop and implement personalized rehabilitation strategies that incorporate behavioral health interventions, social work support, and community resource coordination. Collaborate with multidisciplinary teams—including physicians, nurses, social workers, and addiction counselors—to ensure holistic patient care. Manage clinical case documentation accurately within Electronic Health Records (EHR) systems while maintaining strict adherence to HIPAA regulations for patient privacy. Provide mental health counseling and behavioral therapy tailored to diverse populations, including children, adolescents, and adults with complex conditions. Facilitate discharge planning by coordinating post-acute care services, outpatient programs, and community resources to support ongoing recovery. Participate in utilization management processes to optimize patient care pathways and ensure appropriate resource utilization across inpatient and outpatient settings. Requirements Bachelor's degree in Social Work, Psychology, Nursing, or a related healthcare field; Master's degree preferred. Valid licensure or certification in mental health counseling or social work (e.g., LMSW, LCSW, LMHC) is highly desirable. Proven experience working in hospital settings such as Level I Trauma Centers or Level II Trauma Centers with acute care exposure. Demonstrated knowledge of medical terminology, physiology, and diagnostic evaluation techniques relevant to behavioral health. Experience working with individuals with disabilities or mental health conditions across diverse cultural backgrounds. Strong skills in patient assessment, case management, discharge planning, and community resource coordination. Proficiency in EMR/EHR systems and familiarity with HIPAA compliance standards. Ability to prioritize tasks efficiently while maintaining high-quality patient service and communication skills. Basic Life Support (BLS) certification; CPR certification is required.
