Job Description

Job Overview We are seeking a dedicated and energetic Community Based Rehabilitation Specialist (CBRS) to empower individuals with disabilities and developmental challenges within their communities. In this vital role, you will deliver personalized rehabilitation services, support behavioral health, and promote independence for clients across diverse age groups. Your proactive approach will foster positive outcomes, enhance quality of life, and strengthen community integration. Duties Develop and implement individualized rehabilitation plans tailored to each client's needs, goals, and abilities. Conduct assessments involving vital signs, behavioral observations, and functional evaluations to inform treatment strategies. Provide direct support for clients with developmental disabilities, autism spectrum disorder, PTSD, addiction issues, or other behavioral health conditions. Engage in crisis intervention and crisis management to ensure client safety during emergencies or behavioral episodes. Utilize applied behavior analysis techniques and behavior management strategies to promote positive behaviors and skill development. Collect and document data meticulously on records, progress notes, and intake forms to track client outcomes and inform ongoing care plans. Qualifications Proven experience working with children and individuals with disabilities or developmental challenges. Knowledge of crisis intervention methods, behavior management techniques, and addiction counseling practices. Familiarity with medical terminology, vital signs monitoring, medication administration, and medical documentation standards. Strong background in social work or behavioral health services with experience in trauma-informed care such as PTSD or autism support. Ability to perform comprehensive intake assessments and maintain accurate medical records while adhering to HIPAA guidelines. Excellent communication skills for collaborating with families, caregivers, healthcare professionals, and community partners. Pay:
