Job Description

Franciscan Health Lafayette Central 1501 Hartford St Lafayette, Indiana 47904 Patients who need occupational therapy fall into a wide range of capabilities. Some progress quickly through therapy plans. Others struggle and need modified plans. All of them need hands-on observation and guidance. The Certified Occupational Therapy Assistant (COTA) is the link between the Registered OT and the patient. The COTA is the one who greets each patient with a smile, listens and observes to determine their progress, and maintains records for the OT to review.

WHAT YOU CAN EXPECT
Provide occupational therapy, including selection and use of adaptive equipment. Perform comprehensive occupational therapy treatment and related follow-up services, including discharge planning. Report patient progress and collaborates with evaluating therapist to implement therapy plan of care. Document and submit patient reports and charges within the established timeframe. Exercise clinical judgment following an occupational therapy plan of care. Provide occupational therapy, according to standards of care defined by the state practice act, and other professional and governing agencies. Train and educate patients and family members or others, and document understanding of treatment plan and goals.
QUALIFICATIONS
Associate's Degree
  • Accredited Occupational Therapy Assistant Program
  • Required Occupational Therapist Assistant License (OTA
  • State Licensing Board
  • Required Cardiopulmonary Resuscitation (CPR
  • American Heart Association
  • Required Driver's License
  • Required Auto Insurance
  • Required 1 Year
  • Occupational Therapist Assistant Experience
  • Required 1 Year
  • Home Healthcare Experience
  • Preferred
