Our Psychiatric Rehabilitation Program (PRP) is looking for Rehab Specialists to add to our team of passionate, caring, and reliable Professionals. ABOUT THE POSITION
The Rehabilitation Specialist position provides person-centered support in a team environment to consumers. The focus is on teaching skills both individually and in groups and accessing resources for resiliency, recovery and independent living and success in life. It includes assessing consumers' needs and strengths, assisting with development of the rehabilitation plan, assessing progress, and maintaining the consumers' medical record. Also included is meeting productivity measures and daily documentation requirements. Additionally, the Rehabilitation Specialist is responsible for coordinating psychiatric rehabilitation services, supervising direct care staff and teaching each how to effectively provide PRP services and developing community resources.
More about the duties:
  • Provide oversight for the daily program of rehabilitation services;
  • Provide training and supervision to direct care staff during their initial orientation and throughout their tour of service;
  • Educate direct care staff on how to provide rehabilitation services that correlate to the goals identified on the Individual Rehabilitation Plan and how to appropriately document such services;
  • Conduct Individual Rehabilitation Plan (IRP) at minimum, every three (3) months for children, and every six (6) months for adults
  • Review documentation of services, assessments, IRPs, Monthly summaries, and progress notes;
  • Audit document and maintain clients' clinical records
  • Work in collaboration with the program director and the Administration's ASO to prepare discharge plans
  • Ensure that the program provides rehabilitation activities directed toward development or restoration of skills, including self-care and social skills
  • Ensure that all services are age-appropriate, including group and self-directed leisure activities, both on-site and off-site
  • Co-Facilitate Multidisciplinary Treatment Team meetings
  • Provide referrals to other public and private community agencies upon discharge of client for mental health supports
  • Perform crisis intervention and complete Incident reports, as necessary;
  • Participate in clinical training, treatment team meetings, and staff meetings;
  • All other duties necessary to fulfill program operations
WHAT WE NEED FROM YOU
  • Mental Health professional license ( LGSW, LGPC, LCPC or LCSW-C) ; or
  • -Rehabilitation Counselor certification or
  • Bachelor's prepared certified psychiatric rehabilitation practitioner certified by the U.S. Psychiatric Rehabilitation Association.
  • 24/7 On-call availability for client crisis needs
WHO WE ARE LOOKING FOR
  • At least 2 years of direct care experience working with emotionally disturbed youth, adults and/or families; addictions experience is a plus
  • At least 2 years of supervisory experience
  • A self-starter with excellent client centered and organized approach to client care
  • Excellent organizational skills with an attention to detail
  • Desire to work with a small member team of passionate client-centered professionals
  • High level of integrity, patience, compassion, and care
  • Problem solver and strong team-player
  • Professionalism in dealing with client related pressure, unpredictability, and sensitive situations
  • Maintain professional licensure and/or certification throughout employment with this agency
  • Superb organizational skills and ability to guide, direct, or influence people
  • Excellent written and oral communication skills
  • Able to establish and maintain effective working relationships with mentally or emotionally disabled persons and their families
