Job Description

Cura of Willmar is looking for a compassionate, motivated, and resident-focused Short-Term Rehabilitation Social Worker to join our team! Our Therapy Suites provide high-quality short-term rehabilitation services to individuals recovering from surgery, illness, or injury. As a member of our interdisciplinary team, you will play an important role in helping residents and their families navigate their rehabilitation journey by providing support, education, advocacy, and guidance from admission through discharge. Essential Responsibilities Resident & Family Support Meet with new short-term rehabilitation residents and families to provide orientation and answer questions. Complete psychosocial assessments and document findings in the medical record. Provide emotional support, counseling, and resource referrals as appropriate. Assist residents and families in understanding the rehabilitation process and expected discharge planning. Address resident and family questions or concerns in a professional, timely, and compassionate manner. Care Coordination Participate in interdisciplinary care conferences and daily clinical meetings. Collaborate with nursing, therapy, dietary, activities, admissions, and providers to support resident goals. Advocate for resident preferences while maintaining compliance with facility policies and regulations. Coordinate community resources and post-discharge services as needed. Discharge Planning Coordinate discharge planning beginning at admission. Assist with arranging home health services, durable medical equipment (DME), and community resources. Collaborate with therapists, nursing staff, physicians, and families to ensure safe discharge plans. Communicate with hospitals, clinics, home health agencies, and other community partners as appropriate. Ensure discharge documentation is completed accurately and timely. Documentation & Compliance Maintain accurate and timely documentation in accordance with state and federal regulations. Complete required assessments and social service documentation. Maintain confidentiality and comply with HIPAA requirements. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives as requested. Customer Service Promote a positive rehabilitation experience for residents and families. Respond promptly and professionally to resident and family needs. Foster positive relationships with referral sources, community partners, and facility staff. Requirements Bachelor's degree in Social Work required. Minnesota Social Work licensure preferred. Previous experience in skilled nursing, rehabilitation, healthcare, or case management preferred. Knowledge of Medicare, post-acute care, discharge planning, and community resources preferred. Excellent communication, organization, and problem-solving skills. Ability to work collaboratively with an interdisciplinary healthcare team. Ability to communicate effectively with residents, families, and team members.
