About Us Since 2003, we've been assisting children, families and adults across North Carolina with their behavioral health needs. Carolina Outreach is a group of highly trained behavioral health professionals dedicated to promoting the emotional and physical well-being of our community in a safe and respectful environment. We provide a comprehensive and integrative array of behavioral health services and supports for people of all ages with varying levels of need utilizing an equitable and inclusive approach. About the Role We are seeking a full-time ACT Psych Rehab Specialist to join our Assertive Community Treatment (ACT) team. This role involves providing community-based, in-home individual and family interventions for clients with mental health and substance use challenges. The focus is on promoting symptom stability, enhancing coping skills, fostering interpersonal relationships, and supporting individuals in achieving their highest level of functioning within the community.
Client-Centered Interventions :
Deliver therapeutic interventions based on the person-centered plan. Provide individual and family counseling, psycho-educational activities, and behavioral interventions. Address daily living skills, work-related services, and leisure-time activities.
Collaboration and Coordination :
Work with clients, families, natural supports, and collateral agencies to achieve treatment goals. Act as a liaison between clients and community resources.
Support and Education :
Provide training and education to parents, guardians, or natural supports to improve interactions with clients. Offer substance abuse treatment and support services to help individuals achieve stability and independence.
Documentation and Compliance :
Maintain accurate and timely documentation of services, including treatment notes, goals, and plans. Ensure compliance with the ACT model as measured by TMACT standards. Deliver counseling, psychoeducation, behavioral interventions, and substance use support based on each client's person-centered treatment plan. Collaborate with clients, families, community partners, and support systems to coordinate services and achieve treatment goals. Maintain accurate documentation, monitor client progress, and ensure services align with ACT program standards and requirements. Qualifications At a minimum, the ACT Psychosocial Rehabilitation Specialist - QP must have QP status according to 10A NCAC 27G.0104, including a Bachelor's Degree in a human services field with more than two years full-time, post-Bachelor

experience with the population served; or be a Substance Use Disorder Professional who has two years or more full-time post-bachelor experience in alcoholism and drug abuse counseling or a Bachelor's Degree in a non-human services field with more than four years full-time, post-bachelor
