• Per Completed Visit Per Visit
• $42 Downtime (pre-approved wait time)
• $32/hour "Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.

Service Area & Schedule Cleveland / Northeast Ohio What You'll Do Provide skilled occupational therapy treatments to home health patients under the supervision of an Occupational Therapist in accordance with the established plan of care Assist patients in improving their ability to perform activities of daily living including dressing, bathing, grooming, meal preparation, and home management Implement therapeutic interventions focused on fine motor skills, coordination, cognitive function, and adaptive techniques Monitor and document patient progress toward established therapy goals Educate patients and families on home exercise programs, adaptive equipment, and compensatory strategies Maintain accurate and timely documentation in the EMR within required timeframes Communicate regularly with the supervising OT and interdisciplinary care team regarding patient status and progress What Sets VNA Apart AI-assisted documentation

Requirements Must-Have:

Active COTA license (Ohio) + current BLS certification Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area Minimum 1 year of clinical occupational therapy assistant experience Preferred /

Nice-to-Have:

Prior home health experience Experience working under OT supervision in a community or home-based setting Familiarity with HCHB or similar home health EMR Experience with adaptive equipment recommendations and home modification assessments
