The clinician is responsible for all aspects of clinical care including psychosocial (updates), treatment planning, group, individual and family counseling and discharge planning when clinically appropriate and/or ordered by the attending provider. Risk and symptom assessments will be completed throughout care. Providing evidence based services with scope of practice and coordinating with the multidisciplinary team to impact positive client outcomes.

Education:

LCDCIII, LICDC, LSW

(MSW), LISW, LPC, LPCC, or relevant licensure

Experience:

At least two-years previous experience in a Behavioral Health setting and at least one-year previous experience in group counseling. Preferred Experience working with substance use disorders and co-occurring severe and persistent mental illness, and trauma. Duties Conducting biopsychosocial assessments in order to gather information and make a recommendation for level of care. Conducts individual and group therapy sessions to educate clients regarding psychological, emotional, or substance abuse problems. Provides therapy treatment procedures according to the treatments plan, communicate, and work with the client to achieve the greatest benefit and resolution. Develops a plan of care with specific and measurable goals, objectives, and interventions defining actions unique to each client's needs. Professional, courteous, and respectful interpersonal communication skills with clients, families, and other staff. Represents the program in a positive manner in the program and within the community. Displays active involvement in treatment planning process. Maintains constant communication with the physician/licensed practitioner when client is not responding the treatments. Actively communicates with clients, families, and outside referral sources. Demonstrates proactive communication with those involved with the client's treatment. Effectively communicates to UR on treatment and LOS issues. Completes appropriate paperwork for clients in a timely, accurate and complete manner. Demonstrates appropriate crisis intervention and de-escalation skills. Acts as member of team with crisis situation occurs, assisting others to ensure client safety. Completes all documentation thoroughly, timely, and in accordance with facility standards. Displays a thorough knowledge of confidentiality. Completes releases appropriately. Demonstrates knowledge of HIPAA regulations and abides by those standards. Demonstrates a working knowledge of all clinical regulations, including CARF, TJC, Medicaid, and state and federal laws. Performs other duties as assigned or directed by supervisor.
