Job Duties:

Provides direct service to assigned individuals who are eligible to participate in the Psychiatric Rehabilitation Services Program. This includes service delivery in both the site-based and mobile programs. Completes all assigned tasks by PRS Director to insure the daily rehabilitation activities of the site-based and mobile rehabilitation services are provided as scheduled. Provides group and individual psychiatric rehabilitation services to assigned individuals. Attends weekly rehabilitation team meetings to discuss consumer progress, rehabilitation strategies and monitors effectiveness and consumer satisfaction with services. Completes accurate documentation procedures for the case records of assigned consumers to include rationale for the service, an individualized, person-specific rehabilitation assessment and plan, reviews and revision to rehabilitation plans every three months and progress notes for each encounter. Participates as a member of each consumer's rehabilitation team with other appropriate mental health treatment and rehabilitative programs as well as medical services and community resources. Insures that periodic rehabilitation progress reports to the referral source and treatment providers are completed with the informed consent of each consumer per application of state/federal regulations. Maintains and provides to consumers a list of community resources which are consistent with a person's culture, related to housing, leisure, legal entitlements, emergency needs and mental health treatment services. Complies with all policies and procedures necessary for the operation of both site-based and mobile psychiatric rehabilitation services as approved by the Office of Mental Health and Substance Abuse Services, Office of Medical Assistance and the U.S. Psychiatric Rehabilitation Association (PRA) to insure program compliance with standards. Complies with referral process, access timelines and managed care authorization procedures to insure timely delivery of service and eligibility for payment. Complies with productivity standards developed to insure the fiscal integrity of site-based and mobile PRS. Attends required, necessary and recommended staff development programs. Performs other duties as necessary and required by the Executive Director or his designee.

Requirements:

1. A Bachelor's Degree in human services related field such as psychology, social work, and sociology preferred. 2. Must be a team player with a high degree of flexibility and creativity. 3. Must have the ability to effectively communicate both verbally and in writing. 4.

All required clearances:

Act 33, Act 34 and FBI. 5. Must be computer literate. 6. Must possess a current valid
