The COTA collaborates with patients, caregivers, physicians, and interdisciplinary healthcare professionals to implement individualized treatment plans that promote functional independence, safety, and quality of life. Responsibilities Provide skilled occupational therapy treatment as delegated by the Occupational Therapist. Activities of Daily Living (ADLs) Instrumental Activities of Daily Living (IADLs) Fine motor coordination Upper extremity strength and function Cognitive functioning Home management skills Fall prevention and safety awareness Energy conservation techniques Educate patients and caregivers on home exercise programs and adaptive techniques. Monitor patient progress and communicate changes to the supervising Occupational Therapist. Recommend adaptive equipment and environmental modifications. Complete accurate and timely documentation in the electronic medical record. Participate in interdisciplinary care planning and team collaboration. Maintain compliance with Medicare, state, and company regulations. Our Occupational Therapists work with patients experiencing: Stroke (CVA) Neurological Disorders Parkinson's Disease Multiple Sclerosis Vestibular Disorders Orthopedic Injuries Joint Replacements Generalized Weakness Fall Risk Dementia and Cognitive Impairments Arthritis Chronic Pain Post-Surgical Recovery Balance and Mobility Limitations QualificationsRequired Graduate of an accredited Occupational Therapy Assistant program. Current Arizona Certified Occupational Therapy Assistant License. Current CPR Certification. Valid Driver's License and reliable transportation. Ability to travel throughout assigned service areas. Excellent communication and clinical skills. Preferred Home Health experience. Experience with geriatric and neurologic populations. Knowledge of Medicare documentation requirements. Experience using electronic medical records (EMR).
