Job Description
Evaluate residents' functional abilities and develop individualized treatment plans in collaboration with the interdisciplinary care team. Provide hands-on therapy to improve residents' self-care, mobility, and cognitive skills. Recommend and train residents in the use of adaptive equipment (e.g., reachers, dressing aids, wheelchairs). Document evaluations, progress notes, and discharge summaries in compliance with state, federal, and facility guidelines. Educate residents, families, and caregivers on strategies to maintain independence and prevent injury. Participate in care plan meetings and contribute to holistic resident-centered care. Supervise and mentor Certified Occupational Therapy Assistants (COTAs) as needed
