San Diego, CA Job Details Contract $34 an hour Qualifications CPR Certification Writing skills Social work First aid English High school diploma or GED Driver's License First Aid Certification Geriatrics Full Job Description San Diego Family Circle Adult Day Health Center, looking for a motivated Certified Occupational Therapist Assistant who enjoys working with a frail elderly population. Hours are great. Working with frail seniors and disabled persons attending day-health care.

DUTIES AND RESPONSIBILITIES A . 1.
The Contractor shall provide Occupational Therapy Assistant Services to participants as designated by the Administrator of the Center, as reasonably required for participant care and for appropriate quality and operation of the Center. 2. The Contractor shall be duly qualified to provide Occupational Therapy Assistant Services within the State of California consistent with the standards of practice for Certified Occupational Therapist Assistant Services. Specifically, the Contractor shall perform the duties outlined in the attached Job Description. 3. The Contractor shall provide the Administrator with the following information: a. Documentation of a current health status, signed by a physician, which includes proof of tuberculosis screening. b. A copy of the current certification or license issued by the State of California, or other appropriate parties. c. Current and updated resume. d. Other information specified by the Administrator. This information must be received prior to services being rendered by the Contractor. B. 1. The Contractor shall provide for all services specified in "Job Description" for Certified Occupational Therapist Assistant Services. 2. The Contractor shall be subject to the qualifications and licensure requirements as set forth by the rules and regulations governing Certified Occupational Therapist Assistant Services, and as outlined by State, County and local regulatory agencies. All providers shall be licensed or certified by the State of California, or other appropriate agencies, to practice under the aforementioned standards and shall be approved by the Administrator. 3. The Contractor shall be governed by and construed in accordance with all laws, regulations and contractual obligations incumbent upon the adult day health care provider. C. 1. The Contractor shall use the Center premises solely for the purpose of providing required or prescribed services to participants and/or staff of the Center. No part of the Adult Day Health Care Center shall be used by the Contractor for the practice of his or her specialty independent of services provided to the Center. The Contractor will not allow the premises to be used in any way which would adversely affect the fire or other insurance upon the premises, or which would conflict with any law or regulation affecting the building, or which would interfere with the smooth operation occurring at the premises. 2. The Center agrees to provide the Contractor with the necessary resources for accomplishing the work outlined in the Subcontract. This includes, but is not limited to, routine office supplies, office space for consultation and necessary materials. The resources provided by the Center shall be those considered necessary after consulting with the Contractor, but shall be at the complete discretion of the Administrator. D. 1. The Contractor is accountable to the Center and reports directly to the Administrator or his/her Designee at the Center. 2. The Contractor shall comply with all applicable policies and procedures of the Center, the California Department of Aging, Department of Health Care Services, All

laws, regulations, Waiver's Special Terms and Conditions and Provider Standards of Participation. California Department Public Health and Department of Health and Human Services. 3. The Contractor shall comply with all applicable policies and procedures of the Center, the California Department of Aging, California Department Public Health and the Department of Health and Human Services. 4. The contractor shall not discriminate based on the basis of race, color, national origin, sex, age or disability. 5. The Contractor shall adhere to the provisions outlined in the Center's Plan of Operation. 6. The Contractor shall comply with the policies of Spectrum Adult Day Health Services, Inc., for San Diego Family Circle Adult Day Health Care regarding gifts and supplemental compensations for Contractors. 7. If the Center is dissatisfied with the service of the Contractor, this shall be immediately brought to the Contractor's attention. Both parties shall, in good faith determine whether the performance of the Contractor could be brought to acceptable levels with counsel and assistance, or whether such service should no longer be provided pursuant to this contract. However, it is understood that the final decision with respect to the provision of service by the Contractor shall be at the sole discretion of the Center and the Administrator. E. 1. The Contractor shall procure and maintain, at his expense, professional malpractice insurance for services provided to the Center under this Subcontract. Liability limits of such coverage shall not be less than $1,000,000 per occurrence. The Contractor shall provide a valid certificate of insurance evidencing that there is adequate coverage for services provided. 2. Each party agrees to hold harmless and defend the other from and against all laws and/or liability of claim of any kind, including expenses and reasonable attorney's fees, caused or alleged to have been caused by its actions or those of its shareholders, officers, Contractors, agents or other persons acting under its control, supervision or direction. F. The Contractor shall provide his/her own Workers Compensation insurance. The Center does not provide Workers Compensation for any Contractors. G. The Contractor agrees to provide Occupational Therapist Services to the staff, on site, participate in the meetings of the Center as prescribed at the discretion of the Administrator or his/her Designee; and provide telephone consultation with Center staff, as needed. The need for consultative activity, either by direct visit or telephone, shall be specified by the Administrator or his/her Designee. A mutually agreed upon schedule will be developed by both parties. H. All elements of the program shall be developed based upon standards of practice and in accordance with the rules and regulations governing Adult Day Health Care Centers, including all

Laws, regulations, waiver STC's and SOP's apply, and those established Spectrum Adult Day Health Services, Inc. for San Diego Family Circle Adult Day Health Care.
ARTICLE II BILLING AND COMPENSATION A. COMPENSATION 1.
The Contractor shall be compensated on a fee for service basis, at a rate of $34.00 per Hour, for services performed pursuant to this Subcontract. 2. The Contractor agrees to record the specific hours (e.g. 10:00A.M. to 12:00 P.M.) worked each day on the time card, as directed by the Administrator or his/her Designee. 3. The Contractor agrees to provide to the Center, by the 5th working day of each month, a memorandum of billing for professional services rendered. 4. Within thirty (30) working days following the receipt of the memorandum of billing from the Contractor, Spectrum Adult Day Health Services, Inc. shall present to the Contractor a check reflecting compensations for the hours of service provided by the Contractor, at the agreed upon rate. The Center may review, evaluate and adjust the compensation from time to time by mutual agreement of the Administrator and the Contractor. 5. The Contractor agrees not to submit bills or charges to any individual or third party payer for services rendered. 6. Nothing in this Subcontract shall limit the Center's right to set the rates for service or to bill any individual or third party payer for services rendered by the Contractor. Documenting services provided on the day that they were provided And other duties as required by the administrator and/or program director. Must work well within an interdisciplinary team, enjoy working with the elderly, have good communication skills and writing abilities. Salary depends on experience.
