skilled nursing facility in San Leandro, CA Responsibilities Provide occupational therapy services to residents under the applicable plan of care, including evaluation, treatment implementation, and progress monitoring. Develop, adjust, and progress therapy plans targeting activities of daily living, mobility, upper-extremity function, and cognitive engagement to promote independence. Document therapy sessions and outcomes accurately in the chart; communicate frequently with the interdisciplinary team and participate in care conferences as needed. Implement modification strategies and assistive device usage, ensuring safety, infection control, and adherence to regulatory requirements. Collaborate with nursing, therapy, social work, and other team members to support discharge planning and goal attainment for residents.
