Your expertise will support a diverse patient population, including pediatrics, geriatrics, and individuals with disabilities, ensuring comprehensive care that promotes recovery and well-being. Responsibilities Assess patients' functional abilities and develop tailored care plans to address their specific needs across various settings such as home health, outpatient clinics, schools, and skilled nursing facilities Assist patients with Activities of Daily Living (ADLs), including dressing, grooming, mobility, and self-care tasks to foster independence Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless transitions between care settings Work closely with individuals with autism and other developmental disabilities to implement therapeutic interventions that enhance communication, social skills, and daily functioning Provide therapy services in acute care environments such as hospitals' ICU units and inpatient wards, utilizing your ICU experience and physiology knowledge Conduct case management activities by monitoring progress, adjusting treatment plans, and documenting patient outcomes accurately Educate patients and caregivers on exercises, adaptive techniques, and safety measures to promote ongoing recovery at home or in community settings Qualifications Valid occupational therapy license or certification in the relevant state or country Proven experience working in home health environments with a focus on post-acute care and skilled nursing facilities Strong background in pediatrics, geriatrics, early intervention, outpatient clinics, and school-based therapy programs Familiarity with medical terminology, anatomy knowledge, and physiology principles essential for accurate assessments and interventions Experience assisting with ADLs and discharge planning for diverse patient populations including those with disabilities or recovering from illness or injury Knowledge of speech therapy techniques is a plus but not required; however, interdisciplinary collaboration is essential Ability to work independently in patients' homes while maintaining high standards of patient care and safety Join us to be part of a passionate team committed to transforming lives through expert occupational therapy.
