Job Description

Renewal Rehab is seeking an Occupational Therapy Assistant to support seniors in skilled nursing and post-acute care settings. In this on-site role, you will deliver compassionate, evidence-based interventions under the direction of an OT to improve safety, functional independence, and quality of life. Responsibilities include implementing individualized treatment plans, documenting in the EMR, collaborating with an interdisciplinary team, and educating patients and caregivers. Responsibilities Provide occupational therapy interventions under OT supervision to improve seniors' functional independence in post-acute and long-term care settings. Implement individualized treatment plans focused on ADLs, mobility, and safety using evidence-based geriatric rehab techniques. Accurately document evaluations, treatments, and patient progress in the EMR in accordance with regulatory and facility standards. Collaborate with OTs, nursing, and interdisciplinary teams to adjust care plans and support quality clinical outcomes. Educate patients, families, and caregivers on home programs, adaptive equipment, and safe techniques to maintain function. Contribute to a patient-centered, respectful, and integrity-driven culture aligned with Renewal Rehab's clinical excellence focus.
