We're hiring passionate Home Health Occupational Therapists (OTs) and Occupational Therapy Assistants (OTAs) in Oakland, Macomb, Wayne and Washtenaw counties to deliver personalized, one-on-one care that helps patients live safely and independently at home. Your Key Responsibilities:
Conduct comprehensive evaluations to assess functional needs, develop individualized treatment plans, deliver evidence-based interventions, and monitor progress to maximize patient independence, safety, and quality of life. Deliver evidence-based occupational therapy in accordance with physician orders and the CMS Conditions of Participation (CoPs). Ensure timely, accurate documentation and maintain compliance with CMS, payer, and agency standards. Collaborate with patients, caregivers, physicians, and the interdisciplinary team to maximize function and independence. Educate patients and caregivers on ADLs, home safety, adaptive equipment, fall prevention, and individualized Home Exercise Programs (HEPs) to promote safety, independence, and functional recovery.. Monitor patient progress, update treatment plans as needed, and communicate changes to the care team.
Requirements What We're Looking For:
Degree from an accredited Occupational Therapy program. Current Michigan Occupational Therapist license in good standing. Home health experience preferred (minimum one year). Strong clinical judgment, critical thinking, and problem-solving skills. Excellent communication, interpersonal, and patient education skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Reliable transportation, a valid driver's license, and current auto insurance.
