Provides Occupational Therapy services by supporting the evaluation and treatment of residents/clients, helping develop and carry out therapy plans, and completing related documentation and communication. Works under the supervision of a licensed Occupational Therapist. Follows established occupational therapy standards, along with all facility policies, procedures, and state regulations. Services are provided in accordance with the mission of Guardian Angels.Job Duties:
Under the direction of the registered and/or licensed occupational therapist, optimizes the physical abilities and skills of patients by assisting in the performance of the following activities: assisting in the screening of residents/clients; implementing treatment for assigned residents/clients; delivering treatment to achieve identified objectives according to the established treatment plan; appropriately documenting progress toward identified objectives and communicating resident/client status and needs to the resident/client, the residents/clients family, the staff and other professionals as appropriate. Assists in the selection of Occupational Therapy techniques/media and in the sequence of activities, adapts techniques/media as indicated by resident performance and following consultation with the Occupational Therapist. No major modification to the treatment plan is made without prior consultation with the Occupational Therapist. Assists in establishing and modifying individual resident/client schedules in conjunction with the Occupational Therapist and other team members. Assists the Occupational Therapist in identifying a problem list, long-term goals, short-term goals, and a treatment plan. Monitors patient's response to intervention and consults with Occupational Therapist if modification is indicated. Adheres to department and resident/client schedules and modifies as appropriate for treatment regime. Adapts treatment under the supervision of an Occupational Therapist to meet the individual needs of the resident/client. Provides/recommends/fabricates adaptive devices, orthotics, or prosthetics. Trains resident/client, family, and appropriate staff in the use or application of the above. Participates in resident/client, family, and staff education. Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient. Adheres to facility policies and procedures regarding documentation of occupational therapy services. Attend in-service training and facility meetings as required. Provides in-service education to the members of the resident/client care team and to members of the community. Assumes responsibility for ongoing continuing education and professional development. Submits billing, payroll, and expense documentation in an accurate and timely manner. Assists in the collection of statistics as directed. Provides treatment in accordance with established standards of practice, department procedures, and efficiency standards. Consistently demonstrates sound judgment in the evaluation, planning, implementation, and follow-up of the resident/client therapy programs. Maintains positive relationships and rapport with coworkers, resident/client, family members, and facility personnel. Assists with and participates in Continuous Quality Improvement Program as required. Projects a positive and professional image at all times. Maintains a safe and clean work area and adheres to facility/corporate safety standards. Complies with all Human Resources policies and policies set forth by the organization. To assist in providing a dignified, respectful, resident-centered environment for residents. Follows universal precautions, facility infection control guidelines, sanitation and safety standards of practice in all work and activities. All duties will be performed in accordance with current standards, guidelines, state and federal regulations. Uphold confidential information as required by Guardian Angels policy and procedure and as related to HIPAA. Uphold the mission, vision, and values of Guardian Angels. Performs all other duties as assigned.Qualifications:
Education/Experience:
Associates Degree in Occupational Therapy from an accredited program and licensed in the state of MN.Experience:
One to three years of prior experience in a rehabilitation setting such as acute care, inpatient rehab, outpatient clinics, SNFS, long-term care, assisted living, memory care, or home health. Strong problem-solving and critical thinking skills are essential. Excellent organizational skills. Ability to multitask, prioritize, and work independently. Excellent verbal and written (read, write) communication skills. Proficient with Microsoft Office 365 required. Net Health software experience required. Familiarity with other healthcare software, such as PointClickCare (PCC), is a plus. Maintains good attendance and punctuality. Ability to work varying hours, including weekends and holidays.Benefits Include:
