About Us A Touch of Care, LLC provides therapy and support services for individuals with intellectual and developmental disabilities (DDA population) across Tennessee. We believe every person deserves access to compassionate, individualized care that promotes independence, mobility, and confidence in daily life. Position Overview We are seeking a dedicated Physical Therapist (Evaluator) to perform evaluations for individuals served through Tennessee's DDA and HCBS programs. This role is ideal for PTs who are detail-oriented, value autonomy, and enjoy making a meaningful difference in the community. Responsibilities Complete comprehensive physical therapy evaluations in home or community settings Develop individualized plans of care and recommendations Collaborate with support coordinators, caregivers, and interdisciplinary teams Document all services in compliance with DDA and Tennessee PT Practice Act requirements Submit evaluations and documentation promptly for authorization and billing Qualifications Active Tennessee Physical Therapist License (in good standing) Graduate of an accredited Doctor of Physical Therapy (DPT) or equivalent CAPTE-approved program Experience working with individuals with intellectual/developmental disabilities (preferred) Strong assessment, communication, and organizational skills Reliable transportation and ability to travel to assigned service areas What We Offer $80-$100 per evaluation (based on tier and complexity) Flexible scheduling — create your own hours Supportive administrative team Opportunity to grow within a compassionate, person-centered agency Call or text directly: 901-413-1247
