$58 Per Point Who We Are Who we are: Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health mode. Our ultimate mission is to dramatically improve patient access to home health care. The COTA role is the front line of that mission: you deliver high-quality functional visits in the home under the OT plan of care, helping patients stay safe and independent. We build technology designed around the way clinicians actually work , so you can spend less time on documentation and coordination, and more time with patients.

What you will do:
Deliver skilled COTA visits in the home under the supervising OT's plan of care.
Execute functional treatment plans:
ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training. Track objective functional progress and communicate updates/concerns to the supervising OT. Coordinate with nursing and the broader care team: escalate changes in condition and safety concerns. Document promptly and thoroughly.
