Job Summary:
Provides comprehensive in-home community paramedic visits to patients in the Virtual Care at Home program under the supervision of the Medical Director. Functions as a member of the healthcare team that examines and provides care to patients on a daily basis. Respond to home emergencies and conduct an assessment and care for patients should they acutely decompensate.
Core Responsibilities and Essential Functions:
Conduct comprehensive in-home visits to patients who are enrolled in the Virtual Care at Home program. Administers patients care that is consistent with state protocols including but not limited to medication administration, IV setup and administration, phlebotomy, dressing changes, and checking vital signs. Conducts patient education to the patient and family about their disease state, condition, and discharge plan. Coordinates with the care team for the care plan of the patient including discharge appointments, medical supply needs, medication needs, and other community resource referrals. Observes, record, and report patient conditions and reactions to drugs, treatments, and/or significant incidents back to the provider and care team. Performs other duties as assigned Complies with all Wellstar Health System policies, standards of work, and code of conduct.
Required Minimum Education:
High School Diploma / GED General Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated. ACLS
  • Advanced Cardiac Life Support or ARC-ALS
  • Amer Red Cross Advanced Life Support or ACLS-I
  • ACLS
  • Instructor BLS
  • Basic Life Support or ARC-BLS
  • Amer Red Cross Basic Life Support or BLS-I
  • Basic Life Support
  • Instructor PARA
  • Paramedic Additional License(s) and Certification(s):
Required Minimum Experience:
Minimum 5 years paramedic services Required or Minimum 2 years
