Job Summary Position responsible for conducting home visits to aid patients with chronic illness management.
Job Description Minimum Qualifications Education:
Completion of a Paramedic training program and required hospital and/or field training. Bachelor's Degree preferred.Licenses/Certifications:
Certified in Basic Life Support, ACLS, and PALS within six (6) months of hire EMT-P by the Texas Department of State Health Services or Licensed Paramedic by the Texas Department of State Health Services Certified as Certified Community Paramedic (CP-C) within one (1) year of hire Experience / Knowledge /Skills:
One (1) Three (3) years minimum experience as a paramedic. Effective oral and written communication skills. Ability to perform duties requiring prolonged standing, walking, stooping, bending, and lifting. Strong knowledge of chronic illness management, emergency response skills, and community healthcare practices. Proficiency in electronic medical record documentation and basic computer skills. Principal Accountabilities Delivers appropriate patient care at a competent level and according to the age/needs of the patient/population served.. Obtains subjective and objective data. Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service. Conduct home visits for patients with chronic conditions post-discharge to provide ongoing monitoring, medication management, and health education. Assess patient needs, monitor vital signs, and identify early signs of potential complications, ensuring timely interventions. Educate patients and families on chronic illness management strategies, including dietary modifications, medication adherence, and lifestyle adjustments. Develop individualized care plans in collaboration with a multidisciplinary team, including physicians, nurses, case managers, and social workers. Act as a liaison between the patient's home and the hospital to coordinate care and identify potential barriers to effective chronic illness management. Maintain accurate and thorough documentation of patient interactions, assessments, and outcomes in the hospital's electronic medical record system. Participate in program evaluations and assist in continuous improvement efforts to reduce readmissions. Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff. Demonstrates commitment to caring for every member of our community by creating compassionate and personalized experiences.
