Health Navigator (Case Manager, CHW, BHPP)

We're looking for our next Health Navigator to help guide patients through the healthcare maze with compassion and purpose. This is an In Person/In Field position.

What You'll Do:

Help patients schedule and attend appointments (including driving them when needed) Keep medications, providers, and care plans organized Serve as the human bridge between patients and the healthcare system Problem-solve around social, safety, and health needs Document all activities in our Electronic Health Record system

What You'll Need:

A big heart and excellent organization skills A valid driver's license, reliable vehicle, and ability to drive in Arizona Experience in healthcare, social work, or community support (formal or informal) CPR/First Aid certification, clean background check, and other required clearances
