Bringing Dignity and Care to Every Patient Journey As a Home Health and Hospice Liaison , you'll serve as a trusted resource in the community, building relationships with healthcare providers and guiding families toward compassionate home health and hospice services. Your efforts will help patients access the care they need while driving growth for our agency. Key Responsibilities Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sources. Educate healthcare professionals and community groups about home health and hospice services and benefits. Serve as a knowledgeable, compassionate resource for patients and families during the referral and admission process. Document outreach activities and maintain accurate records of interactions. Ensure all messaging reflects our commitment to dignity, comfort, and quality care. Collaborate with care coordination teams to support smooth transitions for patients. Monitor community needs and identify opportunities to improve access to home health and hospice care. Requirements Bachelor's degree in Marketing, Business Administration, or related field preferred. Minimum 3 years of healthcare marketing experience; home health and/or hospice experience strongly preferred. Strong communication, relationship-building, and public relations skills. Ability to work independently while maintaining a compassionate, patient-first approach. Demonstrated organizational skills and ability to meet outreach goals.
