Program Description:
Enhanced Care Management (ECM) is a comprehensive, patient-centered program designed to provide intensive care coordination and support to individuals with complex medical and social needs. The program aims to improve health outcomes by addressing physical, behavioral, and social determinants of health.
Position Summary:
The Patient Engagement Specialist is responsible for providing support to the ECM program by conducting outreach to initiate enrollment and completing patient enrollments when the case manager is unavailable and assisting with ongoing care coordination. This role works closely with case managers and other healthcare professionals to ensure comprehensive, patient-centered care for individuals. The Patient Engagement Specialist will play a key role in improving patient engagement and facilitating access to ECM services.
Requirements:
Key Responsibilities:
Conduct outreach to potential ECM patients, explain program benefits, and initiate the enrollment process. Complete patient enrollments in accordance with program guidelines when the case manager is unavailable. Support case managers by assisting with administrative tasks, documentation, and patient communication. Provide care coordination services, including scheduling appointments, facilitating referrals, and ensuring follow-up on care plans. Collaborate with healthcare providers, social service agencies, community resources, and both internal and external partners to address patient needs. Conduct home visits and meet patients in the community to complete outreach, enrollments, and provide care coordination support. Maintain accurate and timely documentation of patient interactions, enrollments, and care coordination activities. Monitor patient progress and assist in addressing barriers to care by connecting patients with appropriate services. Participate in team meetings, case reviews, and quality improvement initiatives. Ensure compliance with organizational policies and regulatory requirements.
Qualifications:
High school diploma or equivalent Spanish proficiency Experience in patient outreach preferred. Strong interpersonal and communication skills with the ability to engage patients from diverse backgrounds. Ability to work independently while collaborating effectively within a multidisciplinary team. Proficiency in Microsoft Office Suite and experience with electronic health records (EHR) systems preferred. Strong organizational skills and attention to detail.
Work Environment:
Primary work location is in a healthcare clinic/office setting. May require occasional travel to patient homes, healthcare facilities, and community sites.
Other Requirements:
Possession of valid driver's license and proof of state-required auto liability insurance
Travel:
Up to 20% travel may be required
