At Stallant Health & Wellness, we serve a diverse population, including many Partnership HealthPlan of California and Medi-Cal members. We recognize that a patient's health is determined by much more than what happens inside an exam room. We are seeking a dedicated Case Manager to join our Population Health team. In this role, you won't just be managing files—you'll be removing barriers to housing, transportation, and complex paperwork to ensure our patients receive the dignity and care they deserve. Patient:
You are their dedicated ally and navigator in a confusing system. For the
Provider:
You alleviate rural burnout by handling the social complexities that hinder medical progress. For the
Practice:
You help us remain the "Provider of Choice" by improving quality metrics and building a reputation for holistic care.
Key Responsibilities Transitions of Care:
Monitor hospital/ED discharges; ensure follow-up visits within 7 days and conduct medication reconciliation for home recovery.
SDoH Advocacy:
Identify and solve barriers like food insecurity, housing instability, and transportation.
Medi-Cal Enrollment:
Proactively manage renewals to prevent coverage lapses and partner with the Social Services Department to streamline enrollment.
Clinical Support:
Troubleshoot medication access (prior authorizations), track high-priority specialist referrals, and lead "No-Show Recovery" efforts.
Population Health:
Maintain high-risk registries and conduct gap-closure outreach for preventive screenings and chronic disease follow-ups.
CalAIM Innovation:
Lead the research and development of the CalAIM reimbursement structure to ensure the long-term sustainability of our case management initiatives.
Who You Are Experienced:
2-3 years in a healthcare setting, with a deep understanding of Medi-Cal and Partnership HealthPlan systems.
Educated:
Bachelor's degree in Social Work, Nursing, Public Health, or equivalent clinical experience.
Knowledgeable:
Familiar with CalAIM, Managed Care systems, and local community resources.
Empathetic & Multilingual:
Exceptional communication skills are a must. Bilingual (Spanish/English) candidates are strongly preferred.
Collaborative:
A team player ready to work across departments—from the Referrals Manager to the Medical Director—to implement "Mind, Body, and Spirit" care.
