Full Job Description Position Summary :
Reports to the Peer Restorative Coordinator, the primary responsibility of the Care Navigator is to ensure organizational effectiveness by providing leadership for the development and sustainability of the organization's programs and partnerships.
POSITION WILL BE ONLY FROM MAY 2026-DECEMBER 2026.
Essential Functions/Responsibilities Work with multidisciplinary team to provide support to client's in-custody as well as immediately following their release from custody within various Orange County jails and juvenile custody facilities. Maintain an ongoing caseload of up to 65 clients engaged in mentoring services and court advocacy, including linkages and to community resources; help securing documents like driver's license, identification, services; court mandated classes; tattoo removal; job search services; skill building such as resume writing; enrollment in education and/or vocational programs; and enrollment in college, certification programs, and other long-term/wait list programs for youth and transitional aged youth who have been recently released from incarceration and re-entering the community. Provide one-on-one mentoring with clients during release hours within the program home-base setting. Help clients with system navigation post-release including conducting follow-up phone calls the day after release and conducting site visits with team members to housing placements to ensure they are appropriate and or to ensure client is adjusting back into community with family/caregiver placement. Transport clients as needed to appointments (i.e. Probation check in, arrive to housing placement) within the post-release and for community integration opportunities with support from multidisciplinary team. Work with YTP staff to ensure clients seamlessly transition to ongoing support. Will support team caseload by participating in team check-ins, consultations, and treatment planning to be prepared to provide behavioral health recovery interventions as guided by the treatment team. Support facilitating weekly groups as assigned with other YTP staff members. Prepare and submit weekly case notes to the Peer Restorative Specialist Coordinator. Participate in trainings, outreach events, perform assign duties, and submit timecards/leave requests according to deadlines. Commit to the agency's Mission, Vision, and Values; represent actions that define the overall philosophy and objectives of the agency. Must work a flexible later evening shift (ranging from 9AM-5:30PM to 10:30AM-7PM) and weekends as needed to support the program and leadership. And other duties as assigned. Education and Experience Must have lived experience with incarceration. Must be familiar and/or comfortable using a trauma-informed approach. Bilingual in Spanish or other threshold languages is preferred. Other Qualifications Must have the ability to work both independently and as a member of a team. Must be willing to learn and maintain a positive attitude. Must adhere to policies regarding professional ethics, appropriate boundaries, and client confidentiality. Ability to demonstrate respect for cultural, gender, and lifestyle diversities of clients and staff.
Background Check Requirement:
The successful candidate for this position will be required to pass or clear two background checks. The first background check (Livescan) will be conducted by Project Kinship as per our policies and procedures. The second background check (OC Probation Background clearance) will be conducted by Funders through our contracted vendors, after the candidate is hired with Project Kinship, in accordance with our contractual agreements. Failure to pass either of these background checks may result in the withdrawal of the job offer or termination of employment. Tuberculosis (TB)

The successful candidate for this position will be required to provide documentation of a clear TB (tuberculosis) test or shot, in compliance with Project Kinship's policy.
