This position coordinates care across clinic services, social services, and community-based providers to guide patients to receive comprehensive, integrated, and person-centered care. The Patient Navigator helps patients access services, overcome barriers to care and services. This is a newly created position. While the responsibilities listed reflect our current priorities, the scope and duties of this role may evolve over time to meet the changing needs of the organization. Key Responsibilities Conduct patient outreach and engagement. Coordinate appointments and referrals among primary care, behavioral health, specialists, and other Davis Street and community providers. Assist with follow-up and care transitions. Collaborate with Davis Street clinic and program leaders to support patient needs with Davis Street staff and healthcare providers. Support patient and coordinate referrals as needed. Facilitate communication and information sharing among providers and other programs. Identify and support high-risk patients requiring enhanced patient navigation. Educate patients on Davis Street programs, services and community resources. Advocate for patient needs and access to services. Build partnerships with Davis Street program leaders, community agencies and service providers. Connect patients to housing, food assistance, employment, transportation, financial assistance, and other supportive resources. Maintain accurate and timely documentation. Track activities and quality outcomes. Ensure compliance with HIPAA, confidentiality standards, and behavioral health privacy regulations. Associates or Bachelor's degree preferred. Experience in healthcare, case management, or social services. Knowledge of community resources, healthcare systems, and integrated care models. Strong communication, organizational, and problem-solving skills. Skillful with many facets of communication and interaction with patients including; telephone techniques, etiquette, tact, professionalism and cultural sensitivity while providing a positive and supportive experience. Adheres to privacy, confidentiality, and security policies and procedures related to patient Protected Health Information (PHI). Proficiency with Windows-based software, Microsoft Word, Excel and Outlook. The flexibility to work at designated outreach events. Commitment to the mission and values of Davis Street.
