$29.32 to $35.83 hourly About CommuniCare+OLE Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY/OVERVIEW
The BH Program Navigator is a member of the Integrated Behavioral Health team and works in continuous partnership with the patient, their family when appropriate, IBH providers, CommuniCare+OLE care team, and relevant community partners to improve health outcomes and meet the patient's desired health and wellness goals by: Processing and closing the loop on internal and external behavioral health referrals. Supporting patient access to behavioral health care and real time screening for virtual appointments Serving as the point of contact, advocate, and informational resource for patients, family/caregiver(s), primary care team, payers, and community resources Facilitating access to internal and external services Identifying and addressing gaps in care CommuniCare+OLE provides an inclusive workplace that promotes and values diversity and life experience. MINIMUM POSITION REQUIREMENTS
EDUCATION, EXPERIENCE, SKILLS/TRAINING Education:
High School Diploma or equivalent required. Undergraduate degree in social services preferred.
Experience/Lived Experience:
Experience working with diverse populations displaying respect and sensitivity to diversity, equity and inclusion preferred Experience working in a community health setting preferred Experience working with complex patient situations a plus
Special Skills/Training:
Strong verbal and written communication skills required; Bilingual (English/Spanish) required.
Strong collaborative skills:
ability to effectively and appropriately engage members of the care team, other clinic departments, leadership, and external community partners. Exceptional communication skills; ability to effectively engage patients and assess needs and concerns to ensure that the patient is referred appropriately. Excellent in-person and phone customer service skills; able to manage difficult and emotional patient situations and to respond promptly to patient needs. Ability to adapt to change; approach challenges with an innovative and proactive attitude; and investigate resources. BLS Certification Required.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES 1.
Processes and closes the loop on internal and external behavioral health referrals. 2. Provides support to help maximize quality measures 3. Serves as point of contact, advocate, and informational resource for patients, family/caregiver(s), primary care team, payers, and community resources 4. Facilitates patient access to appropriate behavioral health providers and services, both internal and external 5. Contacts scheduled initial IBH appointments to assess for readiness for appointment 6. Minimizes missed opportunities by managing the IBH schedules and patient wait list 7. Develops and maintains a relationship with community partners to ensure robust collaboration. 8. Participates in huddles, team meetings, and case conferencing as needed. 9. Helps facilitate warm handoffs at all sites 10. Collaborates with care coordinators to limit overlap and ensure closed-loop IBH referrals. 11. In Napa County, Coordinates workflow for Napa County MH Access co-located clinician and CCOLE patient 12. Works as back-up for IBH Program Navigator and Psychiatric MA 13. Documents timely and efficiently in the EMR 14. Determines the appropriate level of IBH services needed to support the patient at any given point in time 15. Works as back-up for other IBH Program Navigators and Psychiatric MAs 16. Attends appropriate training courses/webinars and meetings 17. Gathers and reports data related to behavioral health when needed 18. Provides feedback for the improvement of Integrated Behavioral Health processes 19. Performs other duties as assigned.
