Full-time How you'll make an impact in this role: Coordinate access to health and social services for clients, with a focus on maternal health, public health, or social service program expertise. Conduct initial needs assessments, provide informal counseling and education, and ensure follow-up to guide clients to appropriate resources. Build trust and rapport with patients through strong communication skills and perform program outreach to identify and engage potential clients. Maintain accurate client records and develop strong working relationships with internal teams and external community partners. What minimum qualifications you'll need:Education:
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required. Life atAscension:
