Nurse Patient Advocate Your Nurse Champions Naples, FL $75 an hour - Part-time $75 an hour - Part-time Patient Advocacy Going through a medical crisis is overwhelming, and both patients and their families often get lost in the shuffle. We don't just support patients while they are in the hospital, we walk alongside them and their loved ones long after they leave. As a Patient Advocate, you are their dedicated healthcare quarter back.. You will fight for them in the hospital, ride alongside them to appointments, and help the entire family unit manage life back at home. We need someone with deep clinical roots who can seamlessly translate complex medical words into real world, compassionate care for everyone involved.
What Your Days Look Like Provide Continuous Support:
Guide patients and families through hospital stays (ICU, PCU, ER, Behavioral Health) and continue advocating for them out in the community.
Be Their Appointment Wingman:
Book medical appointments and attend them with the patient and/or family to ensure everyone understands the care plan. Coordinate the

Line up the follow-up care the family needs to support their loved one, from caregiver services and living arrangements to physical and occupational therapy support. Navigate the
Tough Stuff:
Comfortably guide patients and families through sensitive end-of-life care discussions and transitional care.
Teach & Empower:
Educate patients and/or families on managing chronic diseases, breaking down lifestyle changes, diet, nutrition, and medication management/reconciliation so they feel confident at home. Handle the
Logistics:
Manage hospital intake, case management, care coordination and charting via Practice Better , all while keeping privacy (HIPAA) locked down. Grow the
Network :
Attend local networking events to connect with valuable referral partners, and represent us at "Lunch and Learn" events to spread the word about how we serve families.
Deliver Hands On Clinical Care:
Perform skilled nursing tasks out in the field as needed, including dressing changes, managing IV infusions, and other clinical interventions.
Who You Are A Seasoned Registered Nurse:
You have 10+ years of experience in critical care nursing or other intense hospital environments (ERs, ICUs, or ORs). You know how the system works.
A Holistic Advocate:
You have a background in social work, home health, case management, or discharge planning. You understand that health doesn't stop at the hospital exit and impacts not just the patient, but the whole family.
A Skilled Educator:
You know medical terminology inside and out, can confidently reconcile medications, and know how to coach patients and families on diet, nutrition, and lifestyle changes.
Unshakeable and Deeply Kind:
