At NIVA Health, we're looking for a Wellness Case Manager to help guide patients through their wellness journey — from their first interaction with our clinic through ongoing programme engagement. This role is ideal for someone who thrives in a people-focused, fast-paced environment and enjoys combining relationship-building, patient support, organisation, and measurable performance goals. You'll work closely with providers and clinic leadership to help patients stay engaged, informed, and connected to the services designed to improve their overall wellness experience. What You'll Be Doing Building strong, trust-based relationships with patients Supporting patient engagement and wellness programme participation Guiding patients through provider-directed services and next steps Coordinating follow-ups, scheduling, and patient communications Helping create a seamless, elevated patient experience Tracking personal KPIs related to engagement, follow-up activity, and conversion support Supporting new wellness initiatives and service rollouts within the clinic Working closely with providers and clinic staff in a collaborative environment You'll Do Well In This Role If You… Are naturally personable, energetic, and confident Communicate exceptionally well and build rapport easily Have a polished and professional presence Enjoy helping people while working toward measurable goals Are highly organised and proactive with follow-up Can manage multiple patient interactions without losing attention to detail Thrive in a performance-driven but supportive environment Stay calm, solution-focused, and adaptable in fast-moving situations Ideal Backgrounds We'd love to hear from candidates with experience in: Wellness clinics Healthcare coordination Concierge medicine Patient engagement Hospitality or luxury service environments Client success or account management Sales support or relationship management Healthcare experience is helpful, but strong communication skills, professionalism, emotional intelligence, and relationship-building ability are equally important.
