Job Description

The Patient Navigator evaluates psychiatric patients in the emergency department and makes recommendations to the emergency department provider as to the disposition of those patients. This position will consult with the attending psychiatrist. This position also evaluates patients throughout the organization as requested or urgently, who are in need of mental health assessments. This position facilitates or recommends appropriate referrals or preventative services, treatment and support for populations with alcohol, drugs and/or substance abuse. Minimum Education Master's Degree Health Care Administration, Science, Social Work Required or Doctorate Psychology Required Minimum Work Experience 2 years Experience working with children over 5, adolescents and adults who have mental health, substance abuse and/or co-occurring disorders or developmental disabilities. Required Licenses and Certifications BLS for Healthcare Providers (BLS) American Heart Association Upon Hire Required Non-violent crisis intervention (CPI) within 90 Days Required Ukeru User Certification (UKERU_CERT) within 90 Days Required Required Skills, Knowledge and Abilities Skilled in the assessment, diagnosis, and disposition of Behavioral Health patients. Knowledgeable about services available to behavioral health patients. Computer skills. Ability to multitask. Good organization and communication skills (Medium proficiency) Customer driven. Ability to handle confidential information. Strong communication and organizational skills (High proficiency) Talking to others to convey information effectively. Knowledge of medical terminology and records management. Interview, gather, and interpret clinical data for clients to provide a thorough analysis of the client's current state of being. Use interpretive summaries and assessments to provide a clinically supported and valid Diagnostic and Statistical Manual of Mental disorders (DSM) diagnosis Continually assess treatment and recovery progress, and, in consultation with the client and significant others, make appropriate changes to the treatment plan as necessary to ensure progress toward treatment goals (FTPR, progress notes, changes to treatment plan as indicated by client change). Adapt evidence-based practice to the individual needs of the client (EBP include but are not limited to, solution focused brief therapy, other brief therapies, assessment and treatment of co-occurring disorders, CBT, DBT, motivational interviewing, etc). Recognize and manage transfer of care and other critical points in treatment (critical risk points include but are not limited to: initial contact; change or transfer of care; change in legal status; change in life events; change in mental status; change in physical condition; change to a less restrictive level of care; discharge from services). Complete and maintain documentation and productivity to agency and program standards, including those related to medical necessity.
