Job Description
Eckerd Connects seeks an Aftercare Resource Navigator to support youth involved with the Department of Juvenile Justice as they transition back into the community. In this role, you will assess needs, develop individualized aftercare plans, and connect youth and families to education, employment, housing, behavioral health, and community resources. You will coordinate with probation officers, schools, service providers, and families, monitor progress, and adjust plans to reduce recidivism. The Navigator maintains accurate documentation, provides outreach and coaching, and promotes a strengths-based, trauma-informed, culturally responsive approach.
