Patient Navigators work in a variety of settings—community, hospital, home, primary care, and tertiary care. It is their top priority to make their patients feel comfortable and guided throughout their journey.

Bachelor's degree in healthcare related field preferred 2+ years of experience working in a medical setting with oncology patients and families. Ability to prioritize to work and work independently

  • 4:30p As a Patient Navigator , you'll play a vital role on the care team
  • supporting and connecting with patients, survivors, families and healthcare providers, to ensure a seamless, compassionate experience every day.
As a key member of the ambulatory care team, you will: Coordinate appointments with providers for a timely delivery of diagnostic and treatment services. Monitor patient satisfaction with their cancer experience and works with team(s) to address any identified issues or barriers. Advocates on behalf of the patient and empower the patient to self-advocate. Ensures that appropriate medical records are available at scheduled appointments. Arranges transportation assistance with available resources.
About This Location:
Advocate Christ Medical Center Hematology/Oncology in Oak Lawn, IL, delivers comprehensive cancer care to patients across South Chicagoland. Our multidisciplinary program supports patients through the full cancer continuum, including diagnosis, infusion therapy, survivorship, and coordination of complex treatment plans. Our team provides care for a broad range of hematologic and oncologic conditions, offering services such as chemotherapy and immunotherapy administration, care coordination, patient education, and symptom management. We work closely with surgical oncology, radiation oncology, and specialty services to ensure seamless, patient-centered care. Located on the Advocate Christ Medical Center campus, our clinic offers a collaborative, high-acuity environment with strong interdisciplinary support and a commitment to quality outcomes and compassionate care. Physical Requirements and Working Conditions :
Ability to work independently. Ability to handle stress and high workload volumes. Ability to communicate denial of services to members and providers. If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
