CARE Team provides assessments throughout the Deaconess Health System both in person and virtually. CARE Team has been utilizing Telemental Health Assessment technology since 2013 to provide our crisis assessments to our own campuses as well as contracted providers in the area. Team members are expected to consistently perform in a manner that exemplifies Deaconess Cross Pointe standards in both the area of Employee Relations as well as Customer Relations. What you will do Demonstrates clinical competence in the assessment of clinical needs for both psychiatric and substance abuse patients. Documents written information in a professional and timely manner. Maintains verbal communication that is clear, concise, and understood. Complies with organizational requirements for staff development, training, meetings, and in-services. Establishes and maintains effective working relationships with administrative services, CARE Team co-workers, and colleagues within the hospital system. Establishes and maintains positive relationships with patients, community physicians, allied health professionals, and all community customer and referral sources. Assists in obtaining insurance precertification for treatment of Deaconess Health System Behavioral Health patients. Keeps up to date with information related to payors. Maintains familiarity with psychological and sociological and medical terminologies and utilizes appropriately. Maintains awareness of all Deaconess Cross Pointe programs as well as other community resources. Advises supervisor of any issues related to triage or patient disposition. Utilizes therapeutic skills to establish rapport with a wide variety of psychiatric populations. Maintains up to date knowledge of emergency detention procedures and documents accordingly. Prioritizes need for medical screening of patients and seeks consultation immediately when medical issues are identified. Utilizes correct screening procedures such as using breathalyzer. Utilizes general understanding of medical conditions to correctly screen for appropriateness of admission to a psychiatric unit. Provides handoff information in a thorough manner to Care Team staff members, unit staff, and physicians. Maintains constant awareness of activity and assists wherever necessary. Maintains a professional manner in working with coworkers, patients, and outside agencies. Utilizes problem solving skills and time management skills to address issues as they occur with regard to patient admissions. Education Qualifications Master's degree of Social Work (MSW) or master's degree in counseling Required Experience Qualifications Minimum of one-year relevant experience in assessing and working with psychiatric and substance abuse patients Preferred Licenses and Certifications License of Social Work (LSW) or equivalent in appliable state within 6 months of hire Required Valid drivers license and have availability of his/her own transportation for assessments at offsite locations Required Must maintain de-escalation training in accordance with Deaconess Cross Pointe guidelines Required
