Job Description
The Care Management Associate (CMA) will work directly with our high-priority accounts to design and implement workflow designs for program managers. This position aims to enhance the Patient Experience by: (1) Training patients on how to deliver great care management through Phamily's platform (2) Reviewing clients' activity reports to spot urgent problems (3) Proactively recommending care management improvements Job Function To provide patients with Phamily's tried and tested clinical and program practices to ensure overall patient satisfaction, program performance and future growth.
Responsibilities include:
- Communicate with patients via Phamily platform
- Engage weekly with auto suggested and custom messages
- Reach out to physician with all questions from the patient and relay all information back to the patient
- Create care plans for patients utilizing the EMR system
- Review high priority messages and respond accordingly
- Daily outreach/engagement with key care management team members of high-priority accounts (feedback loop with care teams)
- Proactively identify potential un-enrollments due to poor care management or lack of engagement
- Create/strategize bespoke engagement processes to drive increased patient engagement, care, and reimbursement
- Identify opportunities to improve clinical outcomes and impact through Phamily platform
- Promotes communication, both internally and externally to enhance the effectiveness of CCM services
- Provides support services to team members by researching information and assisting in solving problems
- Protects the confidentiality of patient information and adheres to company policies regarding confidentiality.
- Maintain excellent relationships with patients
