These programs are considered resources for the patients in accessing services related to their health care needs.
Programs such as:
Patient Medication Assistance Program, State Medicaid Eligibility, Federal Marketplace enrollment, and other eligible state and national programs In addition, provides community outreach with non-health center patients in order to assist them in accessing programs and services within the community. Functions effectively as a team member and identities opportunities of improvement in performances and work processes. Remains current in knowledge and certified in Assister enrollment programs as required. Works collaboratively with internal team members and external contacts and vendors such as Pharmacies and other resource agencies in the various outreach/benefits programs.
REQUIREMENTS
Arbor Family Health is a Federally Qualified Health Center (FQHC) serving Pointe Coupee, West Baton Rouge and Iberville Parishes.
