Performs the functions of assessing, planning, implementing, and evaluating individualized care of patients utilizing all resources available. The oncology navigator provides education and information to the patient and family, helping to make the care seamless, continuous, and comprehensive.
Responsibilities:
Serves as a central point of contact for the patient to cancer treatment services. Facilitates the patient's access to the healthcare system. Assess the patients' needs and identify barriers to care. Work to remove barriers to ensure patients receive timely and effective care. Advocate for patients within the cancer center, ensuring they are actively involved in treatment planning and decision-making processes. Work closely with the multidisciplinary team, including oncologists, nurses, therapists, and support staff to provide comprehensive care. Track patients' progress throughout their treatment and follow up to ensure adherence to treatment plans. Provide emotional support to patients and their families. Connect patients with needed services or support group.
