Care Coordinator 3.6 3.6 out of 5 stars 59101 Amber Street, Slidell, LA 70461 Children's International Medical Group 35 reviews We are looking for a compassionate, organized, and patient-focused Care Coordinator to join our healthcare team. This role is responsible for helping patients stay engaged in their healthcare by scheduling appointments, completing outreach for preventive care, addressing care gaps, and ensuring families receive the services they need. The ideal candidate is friendly on the phone, detail-oriented, and enjoys building relationships with patients and families. Responsibilities Contact patients and families by phone, text, or other approved communication methods to schedule appointments. Perform outreach to patients who are overdue for well visits, immunizations, annual exams, and other preventive services. Work assigned care gaps by identifying patients who need recommended screenings, vaccinations, follow-up appointments, or chronic disease management visits. Assist with scheduling new patient, follow-up, and specialty appointments. Review daily and weekly reports to identify patients needing outreach. Educate families on the importance of preventive care and keeping appointments. Document all patient outreach accurately in the electronic medical record (EMR). Reschedule missed appointments and reduce no-show rates through proactive outreach. Assist patients with questions regarding appointments, referrals, and next steps in their care. Maintain patient confidentiality and comply with HIPAA regulations. Meet established productivity and quality goals for patient outreach and scheduling. Qualifications High school diploma or equivalent required. Previous healthcare, medical office, call center, or customer service experience preferred. Experience with electronic medical records preferred. Excellent verbal and written communication skills. Strong organizational and time management abilities. Comfortable making a high volume of outbound calls each day. Ability to multitask while maintaining accuracy and professionalism. Bilingual (English/Spanish) is a plus. Preferred Skills Knowledge of pediatric preventive care guidelines. Understanding of quality measures and value-based care. Experience working with patient registries or care gap reports. Strong customer service mindset with a compassionate approach to patient care.
