Bridgewater, MA•Old Colony Correctional Center. This position is part of Spectrum's MOUD (Medications for Opioid Use Disorders) program within the
Department of Corrections Schedule:
Monday•Friday, 8:00am-4:00pm Pay rate: $22.90•26.51/hour

Discounts on wide array of services/entertainment nationwide The Recovery Support Navigator is responsible for: Providing clinical case management to engage, educate and support patients seeking treatment upon release and their families to motivate and maintain them in culturally relevant behavioral health services in the community. This engagement begins 120 days pre-release, and the support continues up to 90 days post-release to the community Working independently and as a team Assisting patients with transportation and housing resources and make referrals as needed Working on-site unless they are in the community working with patients. Coordinating necessary treatment for various patient needs and communicating with all referral sources and treatment entities using proper releases of information Working hours required to meet the business needs of the organization and travel to other programs or locations as needed
Qualifications:
High school diploma required and LADC-II preferred Bachelor's degree in human services or related field or equivalent experience in the behavioral health field preferred Case management experience strongly preferred
