The Breast Navigator for the Hoffman Breast Center plays a vital role in guiding patients through the continuum of breast health care, particularly those undergoing screening and diagnostic mammography. This role serves as a critical liaison between patients, families, and clinical providers. This role ensures seamless coordination of care from diagnosis through treatment and follow-up, focusing on improving patient outcomes, enhancing satisfaction, and reducing barriers to care. The Breast Navigator provides logistical assistance to optimize patient outcomes and satisfaction.
Job Description:
ESSENTIAL DUTIES & RESPONSIBILITIES
(including but not limited to):

Patient Navigation & CoordinationServe as the primary point of contact for patients with abnormal imaging results or cancer diagnosis, guiding them through the healthcare system.

Coordinate scheduling for diagnostic imaging, biopsies, consultations, and surgical procedures.

Reviews relevant medical and surgical history with the patient and communicates relevancy to their ongoing surgical evaluation.

Serves as a liaison between patient, surgeon and the primary team.

Facilitates interaction and communication with health care staff and providers to ensure the proper work up is completed.

Track patient progress, interventions, and outcomes to ensure continuity and quality of care.

Maintains and assists with obtaining outside images.

Education & AdvocacyEducate patients and families about breast imaging findings, procedures, and treatment options.

Assist in identifying high-risk patients and refer them to appropriate specialty clinics.

Team CollaborationActively participate in interdisciplinary team meetings and case reviews.

Build relationships with internal departments (radiology, social services, rehab, etc.) and external providers.

Contribute to quality improvement initiatives and data analysis related to patient outcomes.

Contribute to the development of patient care pathways and protocols.
QUALIFICATIONS
Education:
Bachelor's degree preferred
Certifications:
Breast Health Navigator Certification preferred.
Experience:
Minimum 3-5 years of clinical nursing experience in women's health or breast care preferred.
Skills:
Strong communication and counseling skills.

Ability to work independently and collaboratively.

Ability to manage multiple cases and coordinate care across disciplines.

Proficiency in EMR systems and Microsoft Office.

Pay Range:
$27.00 - $37.00The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
