Job Description

Position Summary:
The Medical Assistant (MA) Care Manager is responsible for the panel management of a subset of patients, including scheduling specialty clinics, managing internal and external referrals, patient outreach, and health coaching. The role works closely with RN Care Manager in providing care management and operating the specialty clinics. The MA Care manager, like the CHW care manager, will apply their knowledge of the community resources and culture to assist the patient in identifying their needs and accessing the correct services.
Job Responsibilities include:
Medical Assistant Duties Processes referrals daily and processes lists of patients who need follow up visits. Identifies patients' barriers to care and assists clients in overcoming barriers, including insurance accesses, transportation, and housing advocacy As a Certified Application Counselor, assists with applying for health insurance benefits, and verifying insurance at every visit. Assists patient solve insurance issues. Coordinates appointments with providers and ensures timely delivery of treatment and navigation services. Translates for the providers as needed. Provide health care navigation assistance to patients prescribed medications that require prior authorizations, applications, co-pay assistance and other related applications. Maintains list of patients on treatment and provides support to patients and providers to ensure regular screening is conducted and appointments are scheduled and kept. Maintains ongoing communication with clients and with providers per protocol. Provides program tracking of patients who have fallen out of care and outreaches to engage back into care. Follows-up on linkages/supported referrals including conducting outreach to locate high risk patients to engage them in appropriate care. Provides education about specialty disease at community meetings and events. Maintains a case load of 20-40 clients in highest need of short term navigation. Accompanies clients to medical and/or social service appointments as needed Assists clients with accessing services to address their immediate needs (e.g., transportation, access to temporary housing, food, clothes, and other basic necessities). Completes all necessary assessments, and documents in the EMR or paper record. Assist in data collection, data preparation, and data entry into program database. Accurately complete basic risk assessment forms, lab requisitions, internal Program forms and other reports required by the program Attends inter-departmental meetings in order to better coordinate harm reduction services for patients. Attends continuing education programs and appropriate in-service and staff development sessions on a regular basis. Keeps current with clinic policies and procedures. Attends and participates in staff meetings and maintains an up-to-date knowledge of clinic and nursing issues. On a regular basis participate in ongoing quality improvement activities related to individual, team, and organizational performance improvement. Assist in data collection, data preparation, and data entry into program database. Remains current in medical assistant skills, including completing the yearly MA competencies Follow universal safety precautions at all times. Travel between sites will be required, including all shelters and mobile health unit sites. Schedule may change periodically. Evening hours required. Performs any and all additional duties as assigned.
Qualifications:
Experience Certified in CPR; Renews CPR certification every 2 years. Bilingual (English/Spanish) required . Must be fluent in both spoken and written English Must have adequate keyboarding skills Education Associates Degree in Human Services field or two years of relevant work experience Graduate of a Medical Assistant program (or evidence of education and experience working in a foreign country as LPN or RN).
