As an Access Navigator, you will serve as a key point of contact for individuals entering treatment, helping guide them through services, assessments, and connections to care. Serve as the primary point of contact for individuals, ensuring consistent engagement and follow-through Complete intake processes, assessments, and required documentation within the electronic health record Collect and utilize Social Determinants of Health (SDOH) data to inform care planning Coordinate referrals to internal programs and community-based services Collaborate closely with clinical staff, care managers, and external providers Maintain consistent communication with individuals throughout their connection to services Support navigation into care management and recovery support programs Ensure accurate documentation, tracking, and follow-up of all individuals servedWho You Are:
High School Diploma or GED required; Associate's Degree preferred 1-2 years of experience in a healthcare or behavioral health setting required Knowledge of substance use, mental health, and care management preferred Strong organizational, communication, and customer service skills Ability to work with diverse populations in a compassionate and professional manner Comfortable using electronic health records and standard computer systems Knowledge of community resources and how to access them Flexible, team-oriented, and able to manage multiple priorities We Hire for HEART!
