Position Summary Responsible for coordinating patient care from admission through discharge by completing assessment, planning, implementation, coordination, monitoring, and evaluation of the patient caseload. Ensures care is appropriate based on patient needs and hospital capabilities. Serves as an advanced clinical resource to patients, families, and staff, and works collaboratively with the interdisciplinary team to provide comprehensive, cost-effective care. "Responsible for moving patients from admission through discharge without disruption to their care…" This PRN role includes variable shifts, weekends, and holidays, and may work in both inpatient and ED settings. Education & Experience BSN required (for all new hires after March 31, 2014) Master's degree preferred Minimum 3 years acute care hospital experience required Licensed Registered Nurse in good standing with the State of Michigan Case Management Certification preferred Skills & Competencies Strong verbal and written communication Ability to prioritize multiple demands in a stressful environment Critical thinking, decision-making, and problem-solving skills Effective negotiation and conflict resolution Ability to work independently and remain self-motivated Proficiency with word processing, spreadsheets, and relevant software Key Responsibilities Complete early assessment and identify patients at risk for post-hospital care needs. Conduct interviews, review records, and assess psychosocial needs. Develop, implement, and revise transition care plans with the care coordination team. Round daily on assigned patients and monitor progress. Identify readmissions, determine causes, and communicate interventions to the team. Drive multidisciplinary rounds and ensure smooth transitions between services. Document clear and specific transitional planning based on patient needs and preferences. Coordinate post-discharge services, including MSW, pastoral care, rehab, long-term care, home health, and community resources. Act as a liaison between patients, physicians, ancillary departments, and community services. Ensure appropriate use of community and outpatient resources. Manage discharge plans to ensure appropriate length of stay and quality outcomes. Evaluate outcomes related to LOS, readmissions, patient satisfaction, and financial variances. Track and trend outlier LOS data to reduce variances.

- House Manager Duties (If Assigned) Provide clinical and administrative direction in the absence of unit leaders. Resolve crises and conflicts; assess and intervene in urgent situations. Investigate patient, visitor, physician, and staff concerns and follow up appropriately. Serve as initial contact for atypical events and ensure proper notifications. Coordinate admissions, transfers, and patient placement. Monitor hospital capacity and collaborate with Staffing Office. Conduct rounds across all departments/units. Maintain utilization review and monitor infection control data. Lead Safety Check-In meetings on weekends and holidays. Manage Employee Incident Hotline calls after hours and document appropriately. Participate in After-Care duties as needed. BBC-Specific Responsibilities Collaborate with multidisciplinary teams to create and review standard care plans or behavioral contracts. Assess eligibility for federal, state, and community programs for uninsured/underinsured patients; assist with paperwork and collaborate with Patient Financial Counselors. Serve as a liaison to community programs, attend community meetings, and maintain a network of resources. Work Environment & Physical Demands High levels of mental/visual concentration required. Regular walking, standing, stooping, bending, sitting, and light lifting. Frequent computer use (40-70 percent of the time).
