Assists patients in applying for Medicaid and/or related resources Contacts newly enrolled Medicaid patients through mail and/or telephone to schedule the first appointment. Monitors and coordinates patient services to determine proper appointments are provided. Prepares patient reports to follow patient care requirements. Works collaboratively with medical staff, patients, and insurers. Maintains patient confidentiality by complying with HIPAA standards. Assists in collecting, researching, and tracking patient records to comply with performance measures. Assists in generating and maintaining reports to provide data to managers and auditors. Follow up with patients and providers to establish satisfaction on care received. Collaborates with members of the health care team and other health professionals related to essential job duties. Must obtain and maintain CCA and Navigator certification. Locations (Consists of traveling to/ or in between the following sites) 5635 W. Fort St, Detroit, Michigan 48209
Education & Experience:
High school graduate or GED plus one-year certificate from college or technical school (health care field). Three to six months related experience and/or training; or equivalent combination of education and experience. Previous experience in a health setting is preferred. Special requirements, qualifications, licenses, or certifications: Must be able to pass certified application counselor and navigator.
