What We Offer Clinical Service Navigator (Registered Nurse)

Schedule :

40 hours per week, Monday through Friday 8-5pm

Location :

Thomasville Medical Center Patient Population:

COPD and Heart Failure Patients Serves as primary liaison between the patient and his/her medical team as well as patient advocate and support. Provide referral sources and support services to patients to work towards lowering patients' readmissions. What We're Looking For Education :

High School Diploma or GED, required. 4 Year / Bachelors Degree, preferred.

Experience :

Three to five years of nursing experience, required. Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.

Licensure/Certification:
