Days Position Summary:
Responsible and accountable for the independent planning, organization, implementation and evaluation of Specialty Practice patients. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.
Detailed responsibilities:
PATIENT CARE
•Perform all duties as appropriate per licensure; assist in specialty clinic
PATIENT CARE
•Coordinate all activities of the specialty related to patient care; including working on barriers to care and coordination of care
PATIENT CARE
•Serve as liaison between patient/family and various members of the health care team
PATIENT CARE
•Counsel and teach families regarding medical conditions, diagnostic procedures, medication control, and other interventions
PATIENT CARE
•Coordinate and facilitate the delivery of services to provide optimal care for the patient within the specialty patient areas with specialty surgical procedures
PATIENT CARE
•Ensure organized and accurate patient data available for clinical decision making; serve as primary liaison between clinical and research teams
PATIENT CARE
•Direct the management of patient care caseload pre and post procedures to provide continuity for the specialty patient and their families
PROGRAMS
•Participate in programs and committees related to the specialty or as directed
EDUCATION
•Coordinate professional and public speaking engagements for educational and public relations purposes
TRAINING
•Participate in educational opportunities and workshops associated with the area specialty REPORTS•Provide statistical reports as needed
SCHEDULES
•As directed may rotate on call shifts including holiday, nights and weekends
EVALUATIONS
•Review daily lab and diagnostic work on specialty work-up to assist in the direction of the specialty evaluation
EDUCATION
•Direct and coordinate educational activities for hospital employees regarding specialty surgeries/procedures; assist physician with research projects related to specialty area procedures
PATIENT CARE
•Collaborate with community physicians on the daily care of specialty patient
PATIENT CARE
•Coordinate diagnostic procedures for complications or follow-up that occur after specialty procedures in consultation with physician
PATIENT CARE
•Direct the management of patient care to provide continuity for the specialty patient and their families
POLICIES AND PROCEDURES
•Maintain established departmental policies and procedures, objectives, and quality assurance programs

•Provide input and assist in the formulation of departmental policies and procedures and budget as requested
MEDICATIONS
•Administer medications, including IV medications, as ordered by physician via the Five Rights; document and communicate clinical findings
TESTS AND PROCEDURES
•Assist with special tests and procedures, ensure proper consent has been obtained
COORDINATION
•Collaborate with members of the specialty team about the candidacy of potential patients
COORDINATION
•Collaborate with other members of the patient's care team, including Cancer Center, ENT, Outside Providers, Medical Director, APRN staff; as well as oncology infusion and radiology appointments

•Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner

•Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"

Registered Nurse or Registered Respiratory Care Practitioner CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
Reg Nurse, Reg Resp Therapist or Reg Polysomno Tech Nonessential:
ACLS w/in 6 months of position
