Guides patients in understanding their options and making important decisions about their treatment plans and medical costs; provides emotional support; and serves as a liaison with health care providers.
Full-Time Job Description The Sleep Therapy Navigator is responsible for ensuring structured follow-up and care coordination for patients diagnosed with sleep apnea. This role oversees the patient journey from diagnosis through therapy initiation, adherence monitoring, and long-term treatment success. The primary goal of this position is to close gaps in care, improve clinical outcomes, and support patients in successfully transitioning from diagnosis to sustained, effective therapy.
Key Responsibilities:
Conduct structured follow-up with all patients diagnosed with sleep apnea to ensure continuity of care Coordinate and track therapy initiation for CPAP Monitor patient adherence and engagement throughout the course of therapy Identify and address barriers to treatment acceptance and compliance Serve as a key point of contact for patients, providing education, support, and guidance throughout their treatment journey Collaborate closely with providers, medical assistants, and administrative teams to ensure timely progression of care Track patient progress and ensure appropriate follow-up intervals are maintained Manage outbound patient communication, including calls, texts, and reminders related to care plans and follow-ups Document all patient interactions and updates accurately within the EHR system Escalate clinical concerns or non-compliance issues to providers or leadership as appropriate Support reporting and tracking of patient outcomes, adherence rates, and care milestones Ensure compliance with HIPAA and all patient confidentiality standards
Qualifications:
High school diploma or equivalent required; healthcare-related education preferred 1+ year of experience in patient coordination, care navigation, or healthcare support role preferred Experience in sleep medicine, respiratory care, or dental/medical environments is a plus Familiarity with EHR systems and patient tracking tools preferred Knowledge, Skills, and Abilities Strong communication skills with a patient-centered approach Ability to build rapport and guide patients through treatment plans High level of organization and attention to detail Ability to manage multiple patient workflows simultaneously Problem-solving skills to address barriers to care and adherence Ability to work collaboratively across clinical and administrative teams Proficiency with computer systems, scheduling platforms, and documentation tools Work Environment Clinical office setting Combination of desk work and patient communication (phone, text, digital platforms) Frequent coordination with clinical and administrative staff Schedule Full-time, Monday through Friday May require flexibility based on patient needs and follow-up schedules Physical Requirements Ability to sit for extended periods Frequent use of phone and computer systems Ability to perform repetitive tasks such as typing and data entry Position Impact This role is critical to improving patient outcomes by ensuring no patient falls through the cracks after diagnosis.
