As the Peer Navigator , you will play a pivotal role in individuals' lives, making a significant impact through: Make initial contact with eligible participants within 3 hours of receiving referral Facilitate initial intake with referrals Deliver outreach and engagement services in accordance with the service plan According to program guidelines, consult with case management staff, Health HUB staff and/or participant's providers on progress of service/care plan services Conduct prevention related harm reduction/risk reduction interventions with participants Provide linkage and navigation to needed services for family members and other identified support systems Maintain case records and documentation in accordance with agency and OASAS standards

$ 20.51

Peer Navigator Schedule:
Mon - Fri (37.5 hours/week)
Qualifications:
